
Donna's Success Story
January 2023
Donna was admitted to Norwalk Nursing and Rehab in February 2021 after suffering a stroke and was placed on hospice care shortly after her arrival. In spring of 2022, Donna graduated hospice care and began working with our therapy team with hopes of eating regular food instead of receiving nourishment from her feeding tube. Donna accomplished far more than she had originally hoped for after working with our therapy team. She is now the proud new owner of an electric wheelchair and able to get around the facility by herself.
She has several good friends and eats meals and plays cards with them daily. She enjoys bingo, is a spokesperson for the Resident Council, and is so loved and cared for by the staff at Norwalk Nursing & Rehab. Donna has a wonderful story that we are so proud to share. Great job, Donna!
Marcella's Success Story

Marcella O. admitted to Norwalk Nursing and Rehabilitation Center on November 30th, 2022 under the care of Dr. Michael O’Conner. During the course of the stay, Occupational Therapy, Physical Therapy, and Speech Therapy were initiated to efficiently and effectively identify Marcella’s unique needs and tailor specialized treatment plans to successfully transition to the next level of care. On December 20th, 2022, Marcella successfully discharged home. The rehabilitation length of stay was 21 days.

Harriet's Success Story

Herbert Success Story

On September 06, 2022, Herbert successfully discharged home. Herbert worked hard in therapy and was able to discharge home with home health services. Great Job, Herb!

I was really skeptical when I first came here and had a chip on my shoulder because I wanted to go home. The staff gave me my space to settle in and took the time to listen to me when I wanted talk. Ruth's Success Story

Ruth J. came to Norwalk Nursing and Rehab after a hospitalization to receive therapy prior to returning home. While at Norwalk, Ruth received both PT and OT to increase her strength, safety with functional mobility, safety with ADLs, and to increase gait distance and tolerance. Ruth worked very hard in therapy and was able to return home independently with home health services.

Joan's Success Story

Albert's Success Story
February 2021
Albert recently transitioned to Norwalk Nursing and Rehabilitation Community from assisted living. Albert, over the past few weeks, has been working with our therapy team to improve both physically and mentally. When Albert was asked about how his therapy has been going, he mentioned that the therapy staff are encouraging and very kind. He also mentioned that he feels his walking, standing, and concentration have greatly improved. Overall, Albert states he is feeling much better physically!
Albert shared that he can’t point out any specific staff member who is better than another because they are all good! He feels that all the staff look out for him- even the ones who wake him from his slumber each morning. Albert states that he would absolutely recommend Norwalk Nursing and Rehab!
Christine's Success Story
Christine M., was admitted to Norwalk Nursing and Rehabilitation Center on 8/19/2020. A customized rehabilitation program was implemented to address functional deficits related to the diagnosis of C-Diff. Christine came to Norwalk Nursing and Rehab after developing weakness from C-Dif. She worked hard with her therapies and was able to return home within 3 weeks at an independent level. Christine was discharged from Norwalk Nursing and Rehabilitation Center on 9/11/2020. It was a pleasure serving your patient’s rehabilitation needs.
Gary's Success Story
Gary was admitted to Norwalk Nursing and Rehab on 9/28 after a hospital stay that left him with generalized weakness. Gary received Occupational Therapy and Physical Therapy while he was at Norwalk Nursing and Rehab. A customized rehabilitation program was implemented to address functional deficits related to the diagnosis of Weakness. After successfully completing the therapy program at Norwalk Nursing and Rehabilitation Center, Gary Hadley was discharged to home. Gary worked very hard and was able to discharge home in two weeks with his wife and home health services. It was a pleasure serving your patient’s rehabilitation needs.

Michael's Success Story
Michael N. was admitted to Norwalk Nursing & Rehabilitation Center on 5/4/2020.
Michael came to Norwalk Nursing and Rehab following an infection in his foot after an amputation. Michael was determined to return home and his PLOF. He was treated by PT, OT & ST services and was able to return home within two short weeks.

Ardelle's Success Story
Ardelle K. was admitted to Norwalk Nursing and Rehabilitation Center on 2/27/2020. A customized rehabilitation program was implemented to address functional deficits.
Ardelle came to Norwalk Nursing and Rehab following a fall at home that caused an ankle sprain. She was treated by Physical Therapy, Occupational Therapy, and Speech Therapy. Ardelle worked hard to reach her goals with therapy and was able to return home with home health services within 3 weeks.
After successfully completing the therapy program at Norwalk Nursing and Rehabilitation Center, Ardelle Keeney was discharged from Norwalk Nursing and Rehabilitation Center on 3/11/2020.
